What is UB04?

The UB-04 (Uniform Billing 2004) is a standard claim form used by hospitals, skilled nursing facilities, and other institutional providers to bill Medicare, Medicaid, and other third-party payers for healthcare services. It contains information about the patient, the provider, diagnoses, procedures, and charges. It is now largely replaced by the 837I electronic claim format.
